Fixed Term position to 30 June 2026 with an extension subject to ongoing funding.

Objective
The Integrated Team Care Service aims to improve health and access outcomes for Aboriginal and
Torres Strait Islander people with chronic health conditions.
The ITC Aboriginal Outreach Worker has the primary responsibility of connecting with Aboriginal and Torres Strait Islanders clients and their families to seek to increase the efficiency and effectiveness of health services.
Clients/patients particularly those at risk of poor health outcomes will be assisted to improve coordination of care between mainstream primary care and the Aboriginal and Torres Strait Islander health sector.
Role responsibilities
The ITC Outreach Worker will be responsible for:
contributing to improved treatment and management of chronic conditions for Aboriginal and Torres
Strait Islander people enrolled in the ITC program;
improve access to appropriate health care through care coordination and provision of supplementary
services for eligible Aboriginal and Torres Strait Islander people with chronic disease;
foster collaboration and support between mainstream primary care and the Aboriginal and Torres
Strait Islander health sector; and Improve the capacity of mainstream primary care services to deliver culturally appropriate services to them
arranging the required services as outlined in the patients GP Management Plan such as
accessing allied health practitioners and medical specialists.
educating and providing patients with relevant resources and educational material on their chronic disease to support their better understanding and self-management
disease to support their better understanding and self-management.
assisting patients to adhere to treatment regimens for example assisting with medication
compliance.
assisting patients to connect with appropriate community based services such as those providing support for daily living
assisting patients to participate in regular reviews by their GPs.
co-ordinating specialist allied health and/or specific burden of disease clinics;
accompanying ITC clients to travel to and from health appointments.
conducting home visits and outreach when appropriate;
client health care coordination;
working with a partner GP to co-ordinate and support patients to come back to the clinic for results and/ or follow up care;
assist clients with referrals to external allied health and community service organisations;
identify barriers that may impact on clients access to health services and provide feedback to Senior staff and assist with the implementation of solutions;
work collaboratively within multidisciplinary healthcare teams to achieve better health outcomes for Aboriginal people and communities and play a key role in facilitating relationships between Aboriginal patients and other health professionals; and
promote health assessments as an essential component annual client care.

C Class drivers licence is mandatory for this role.
Qualifications
Aboriginal Health Workers are required to have one of the qualifications below in Aboriginal Primary Health Care:
Certificate II in Aboriginal Primary Health Care
Certificate III in Aboriginal Primary Health Care
Certificate IV in Aboriginal Primary Health Care
Certificate IV in Aboriginal Primary Health Care (Practice)
Certificate IV in Aboriginal Primary Health Care (Community)
Diploma of Aboriginal Primary Health Care (Practice)
Diploma of Aboriginal Primary Health Care (Community)
A Nurse must hold a qualification as an Enrolled Nurse or Registered Nurse.
Workplace responsibilities
Work as a team member seeking support guidance and direction as required;
Attend meetings and undertake training as appropriate to the role of ITC Outreach Worker;
Perform any other duties consistent with the terms of employments as may be directed or implied from time to time; and
Participate in staff performance reviews.
Workplace Health and Safety responsibilities
Follow defined WH&S and injury management policies and procedures;
Take reasonable care for the safety of others in the workplace;
Ensure organisational compliance with any requirements of the WH&S Act and other legislation with regard to health safety and welfare in the workplace;
Not intentionally or recklessly interfere with or misuse anything provided in the interests of health safety and welfare in compliance with the WH&S Act or other legislation;
Report all accidents or incidents which did or could have resulted in injury to others in the workplace or damage to property;
Report all hazards or potential hazards;
Be familiar with emergency evacuation procedures and participate in regular training in safety procedures; and
Provide input into regular safety inspections for their department.
Child Protection Responsibilities
Be familiar with and adhere to legislation in relation to Child Protection and comply with NSW Health Frontline Procedures for the Protection of Children and Young People;
This appointment is subject to a satisfactory clearance for a police check and a clearance for working with Children undertaken by the Office of the Childrens Guardian (NSW).
Continuous Quality Improvement responsibilities
It is the responsibility of each staff member to be aware of the contents of the policy and procedures manuals and work within the principles contained therein.
Each staff member is expected to be committed to quality service and to participate in activities to enhance continuous quality improvements in the workplace.

Salary
Galambila offers the following attractive salary package for the Aboriginal Caseworker Domestic and Family Violence:
A competitive salary package with a base salary negotiable based on experience and the ability to salary sacrifice;
12% superannuation;
Additional 5 days sick leave per year;
Additional leave for NAIDOC and Christmas; and
Additional 10 days paid study leave per year.
