Care Around the Block is a local and family-owned company that provides Care Management to older adults partnering with them in their aging journey. Our objective is to assist our clients and their responsible parties in managing and navigating the issues they may face as they age through the processes of assessment care planning and evaluation of care.
Care Around the Block is in search of LPN/Care Managers who:
Job Summary:The successful Care Manager gets a rush when she/he walks into a seemingly hopeless situation and can help the client and family meet their goals. They are fulfilled by knowing a job is well done. You would be responsible for providing quality professional care management services to all clients and their responsible parties adhering to the ALCA Pledge of Ethics and Standards of Practice and Care Around the Blocks mission statement.
Classification:Non- Exempt Hourly.Part-time positions available.

Work Environment:This job operates in a professional office environment and extends into clients homes. May be required to perform certain duties from a home office.
Travel:Primarily local during the business day although some out- of- area and overnight travel may be expected.
Physical Demands:Must be able to sit for long periods stoop lift and bend effectively to be able to perform the above-listed Job Duties & Responsibilities. Must be able to lift 25 pounds. Must be able to sit in a car and drive sometimes for several hours per day.
