Alfred Health is a leader in health care delivery improvement research and education. We are the main provider of health services to people living in the inner southeast suburbs of Melbourne from ambulatory to inpatient and home and community-based services.

  • Fixed Term position Full time (80 hours per fortnight)

The Team

The Home Therapies Team is part of the Renal Ambulatory Service and delivers acute home peritoneal dialysis and Home haemodialysis service. The service is responsible for providing education training and support to home therapies patients and their families as well as the larger multi-disciplinary and medical teams.

The Role

This role focuses on ambulatory care and inpatient support. You will collaborate with the Home Therapies team to coordinate care for 100 PD and HHD outpatients including assessments training education and testing. Inpatient duties involve supporting established PD patients and providing acute PD care for seriously ill patients. The team is led by a Home Therapies CNC and Nurse Manager supported by a Nephrologist and is part of the renal multidisciplinary team.

Experience required

  • Proven ability to work independently and within teams
  • Strong IT skills especially in data management and Excel
  • Critical thinking skills
  • Advanced clinical assessment skills
  • Sound knowledge of dialysis and renal nursing
  • Ability to influence and support change and patient-centered service delivery
  • Excellent communication skills
  • Enthusiasm and motivation to educate staff
  • Clinical expertise in daily service delivery and exploring new clinical team approaches
  • Experience with high-level PD patient management desirable

Qualifications Required

  • Post Graduate qualifications Renal Nursing preferred
  • Previous PD unit experience essential
  • Holds current Drivers licence
  • Unrestricted registration as a Nurse with AHPRA
