Job Summary:
This role is for a Physical Therapist providing skilled home health services in Medford OR and surrounding areas. The therapist will manage a caseload of up to six patients daily conducting patient assessments developing treatment plans and providing direct patient care. The position requires a minimum of one year of skilled home health experience and proficiency in Oasis documentation.

Responsibilities:
- Provide skilled physical therapy services to patients in their homes.
- Develop and implement individualized treatment plans.
- Conduct patient assessments and document progress in Oasis.
- Manage a caseload of up to six patients per day.
- Maintain accurate and timely documentation.
- Adhere to all applicable regulations and guidelines.
- Manage travel between patient locations within a 50-75 mile radius.
Required Skills & Certifications:
- Current Physical Therapist license in Oregon.
- Minimum of one year of skilled home health experience.
- Proficiency in Oasis documentation.
- Basic Life Support (BLS) certification.
- Valid drivers license car insurance and reliable transportation.
Preferred Skills & Certifications:
- Experience with a variety of patient populations and diagnoses.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
Special Considerations:
- 8-hour day shifts; no 4x10 or 3x12 schedules accommodated.
- Up to one shift per pay period may be called off by the facility.
- Travel between patient locations is required with occasional maximum radius of 50 miles. The system attempts to minimize travel time.
- First-time home health travelers are not accepted.
Scheduling:
- 8-hour day shifts.
- No weekend work required.
- Block scheduling is not accommodated.
