Job Description
Qualifications:
-
Masters degree in Social Work (MSW) from an accredited institution
-
Active Licensed Clinical Social Worker (LCSW) in Alabama
-
Proficiency with EHR systems and documentation
Your Responsibilities will Include:
Direct Patient Care
-
Conduct comprehensive psychosocial assessments & develop individualized care plans
-
Provide short-term solution-focused counseling and behavioral interventions
-
Deliver crisis intervention and safety planning as needed
-
Support patients with chronic disease management through behavioral health strategies
-
Facilitate referrals to community resources mental health services housing or substance use treatment
Care Coordination & Provider Support
-
Work closely with primary care providers nursing staff and case managers to coordinate patient care
-
Serve as a liaison between patients and external support agencies
-
Participate in daily huddles case conferences and interdisciplinary team meetings
-
Assist providers in identifying behavioral health concerns and social determinants of health impacting treatment adherence
Documentation & Compliance
-
Maintain timely and accurate electronic health record (EHR) Athena documentation in compliance with FQHC and state/federal regulations
-
Ensure HIPAA compliance and ethical standards in all interactions
Program Development (Optional/Future Growth)
-
Contribute to program planning and quality improvement initiatives related to behavioral health integration
-
Assist in staff training related to trauma-informed care and social services
