Job Description
We are working closely alongside a Local Authority in Liverpool to assist with the appointment of a Social Care Assessor to work in a residential care setting.
We have both regular shifts (17.50 hours per week) and bank shifts available. Please note that we dont offer sponsorship and the worker should be able to work full time without restrictions. We would need 5 years work references checked out before you start the shifts.
Rate of Pay: 17.80 - 23.59 per hour
Summary:
Liverpool City Council is seeking a dedicated and empathetic Social Care Assessor to support individuals and communities by assessing care needs and helping develop appropriate support plans. This role involves working with adults families and multi-disciplinary teams to ensure effective delivery of care in line with legislative and policy frameworks.
Responsibilities:
- Conduct assessments of individuals care and support needs under the Care Act 2014.
- Work collaboratively with service users families and professionals to co-produce support plans.
- Monitor and review care packages ensuring the outcomes are met and support remains appropriate.
- Provide advice signposting and referrals to appropriate services and community resources.
- Maintain accurate case records documentation and reports in accordance with council policies.
Qualifications:
- NVQ Level 3 in Health & Social Care or equivalent qualification.
Essentials:
- Previous experience in an adult social care or health setting including care assessments
- Experience working in a multidisciplinary environment liaising with other professionals and stakeholders.
- Knowledge of the Care Act 2014 and other relevant legislation and frameworks.
Please note:
- You should be available to work immediately or at a short notice.
- You should have right to work in U.K
- This role requires an Enhanced DBS
