Job Description
Job Title: Adult Social Worker - Warrington
Location:Warrington (Hybrid)
Pay Rate: 30 per hour
Job Description:
Liquid Personnelis currently recruiting anAdult Social Workeron behalf of its client based inWarrington.
What will your responsibilities be
- Adopting a Discharge to Assess approach.
- You will facilitate safe and timely transfer from hospital to home or the community and where an interim community placement is required.
- You will follow the service user building on your initial assessment to plan for service users longer-term needs.

Qualifications & Experience:
To be successful in this role you must have
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
