Job Summary:
This position is for a Registered Nurse (RN) providing skilled nursing care case management and hospice care to patients facing advancing age frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate with an interdisciplinary team. A minimum of two years of experience is required.

Responsibilities:
- Patient assessments and monitoring.
- Medication administration.
- Treatment planning and implementation.
- Collaboration with interdisciplinary team (physicians other nurses therapists etc.).
- Case management.
- Hospice care (if applicable).
- Documentation of patient care.
- Medication administration.

Required Skills & Certifications:
- Active Registered Nurse (RN) license in the state of employment.
- Bachelors degree in Nursing or related field.
- Minimum two years of experience in hospital home health or hospice care.
- Clinical assessment skills.
- Medication administration skills.
- Treatment planning skills.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Drivers license car insurance and personal vehicle for field work (if applicable).
- Bachelors degree in Nursing or related field.
- Minimum two years of experience in hospital home health or hospice care.

Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
Special Considerations:
- This is a full-time evening position Sunday-Thursday 3:30 pm - 12:00 am.
- Rotating weekend night call is required.
- The position is a contract role with a flex agency.
- Travel up to 75 miles from the hospital may be required.
- The work environment is a tertiary care hospital.

- Five 8-hour shifts per week Sunday-Thursday 3:30 pm - 12:00 am.
