Position Summary - Identifies available community resources to assist clients in obtaining needed services and collaborates with professionals from multiple systems to address treatment needs
- Works effectively with community agencies to transition clients from FAIR to other service providers
- Works collaboratively with the client and/or family Mental Health Assessors and other professionals to develop and implement ISP goals
- Employs goal oriented problem-solving techniques to assist clients

- Effective communication skills
- Ability to interact professionally with persons from diverse backgrounds
- Knowledge of the Hamilton County public mental health system and other community resources
