Position Summary
WeCare Medical Specialty Group is seeking a licensed and experienced Remote Social Worker to join our growing behavioral health team. This position plays a critical role in supporting the emotional mental and psychosocial well-being of our patients. The ideal candidate will have experience in telehealth strong case management skills and a deep understanding of patient advocacy and interdisciplinary collaboration.
This role will involve working closely with physicians nurse practitioners and other healthcare professionals to assess patient needs develop care plans and ensure continuity of care through virtual platforms.
Key Responsibilities
Provide virtual psychosocial assessments and therapeutic support to patients and their families.
Develop and implement individualized care plans addressing mental health emotional well-being and social service needs.
Offer counseling and support to individuals facing chronic illness behavioral health challenges or major life transitions.
Coordinate community resources referrals and services including housing food assistance substance use programs and financial aid.
Maintain timely and accurate documentation in compliance with HIPAA and company policies.
Participate in interdisciplinary team meetings and care coordination efforts.
Serve as a liaison between patients healthcare providers and community agencies.
Educate patients and families about coping strategies treatment options and long-term support resources.
Monitor patient progress and adapt interventions as necessary.
Uphold ethical standards and maintain patient confidentiality in a remote setting.
Qualifications
Required:
Masters Degree in Social Work (MSW) from an accredited program.
Active and unrestricted LMSW LCSW or equivalent state licensure in the U.S.
Minimum of 2 years experience in a clinical or healthcare setting.
Strong understanding of behavioral health chronic care and community resources.
Excellent communication organizational and documentation skills.
Proficient in telehealth platforms and remote care technologies.
Compassionate non-judgmental and culturally sensitive approach to patient care.
U.S. Citizenship or valid U.S. Work Authorization is mandatory.
Preferred:
Bilingual (Spanish Mandarin or other languages).
Experience working in a multi-specialty group practice or with underserved populations.
Knowledge of EMR systems such as eClinicalWorks or Athenahealth.
