The Role: Counselor II (LCSW or LCPC)

The Counselor provides assessment diagnostic individual group and family therapy services to patients treated on an outpatient basis.

The Opportunity:

The Work:

• Formulates individual patient treatment plans and completes all required documentation
• Systematically and continually assesses the patients mental status in collaboration with the patient/family/providers
• Provides outpatient individual group and family counseling
• Identifies and refers patients to appropriate community resources for follow-up care

You Have:

Active LCSW or LCPC required
At least 1 year of Behavioral Health experience preferred
Excellent teamwork communication and collaboration skills
Proven and demonstrated critical thinking skills
