Job Summary

About Us

MGB strives to advance health equity improve health outcomes and promote well-being of our primary care patients by addressing health-related social needs system navigation and care coordination as standard of care. Patient Navigators are an integral part of achieving these goals.

Patient Navigator Population Health Colorectal Cancer Screening role is responsible for working closely with patients families and healthcare providers to ensure a seamless and patient-centered care experience by assisting with navigating the complexities of the healthcare system coordinating resources and providing support to improve patient outcomes.

What Youll Do

Patient Navigators play a pivotal role in facilitating access to cancer screening services for the patients we serve. By building trust with their patients Patient Navigators help patients improve access to healthcare. Patient Navigators have the skills and experience to understand their patients circumstances. By walking alongside their patients Patient Navigators help to address medical and psychosocial needs in order to promote self-efficacy help patients meet their goals and improve health outcomes.

Patient Navigators use their unique skills (motivational interviewing trauma sensitive care coaching etc.) to help patients manage their chronic diseases complete preventative health screenings adhere to medications connect to community resources and gain strength and confidence in managing their own health. In addition Patient Navigators will work with patients to help decrease barriers to timely preventative and follow-up care in the midst of challenging social environmental and economic situations.

The Patient Navigator will aid patients in the coordination and completion of appointments inside and outside of MGB. Patient Navigators will work with patients to alleviate social determinants of health as well as improving their overall health and well-being. While the Patient Navigator is not a clinical position it requires a good knowledge (ability to learn) of basic clinical concepts and an understanding of when a referral to a licensed clinician is appropriate.



Qualifications

Key Functions

  • Advocate for patients and their families ensuring their needs and preferences are considered in the care planning process.

  • Serve as a liaison between patients healthcare providers and other relevant stakeholders.

  • Collaborate with the interdisciplinary healthcare team to coordinate patient care services appointments and follow-up plans.

  • Assist patients in understanding and adhering to their care plans.

  • Identify and connect patients with appropriate healthcare and community resources such as support groups financial assistance programs and transportation services.

  • Educate patients and their families about their medical conditions treatment options and self-care strategies.

  • Ensure that patients are informed and empowered to actively participate in their healthcare decisions.

  • Maintain accurate and detailed documentation of patient interactions care plans and resource referrals.

Qualifications

  • Bachelors degree in social work public health education or related field preferred.

  • 1-2 years experience in social work case management or related field ideally in a clinical setting preferred


Skills and Abilities for Success

  • Strong knowledge of healthcare resources community services and patient advocacy.

  • Excellent communication and interpersonal skills.

  • Ability to work collaboratively with diverse healthcare professionals.

  • Strong organizational and time management skills.

  • Familiarity with electronic health records and case management software.

  • Strong commitment to social justice health equity and positively impacting the care of at-risk patients.

  • Experience working in a healthcare environment

  • Proficient in Spanish Haitian-Creole or other language(s) strongly preferred

  • M-F Eastern Business Hours required
  • This is a hybrid role with 70% remote work and 30% work in the community or on-site.

  • Hybrid required Onsite Working days weekly vary must be open to business needs

  • Remote working days require stable secure quiet compliant working station
