The Opportunity
Were looking for a caring skilled Licensed Practical Nurse (LPN) to join our team and support individuals with disabilities in living healthier more independent lives. Youll be more than a nurse-youll be an advocate mentor and trusted companion.
What Youll Do
- Provide hands-on support for daily living including hygiene nutrition and health routines
- Administer medications and treatments per physician orders
- Monitor health conditions and collaborate with RNs and care teams
- Teach and guide individuals to build confidence and independence
- Respond to behavioral needs with calm trained intervention
- Support individual service plans and help each person thrive
- Serve as a positive role model and create a welcoming home-like environment
- Communicate with families guardians and team members
- Attend medical appointments as needed
What Youll Bring
- Licensed Practical Nurse (LPN) in New York State
- One year of experience in a healthcare or direct care setting
- Passionate about helping others reach their full potential
- A team player with strong communication and problem-solving skills
- Cleared through the NY State Child Abuse Registry
- Able to meet Lifetime Assistance vehicle operator requirements
