THP LICSW Job Description

The Transgender Health Program (THP) is an initiative of the Department of Medicine to provide comprehensive gender affirming care to individuals across the lifespan in close collaboration with Mass General for Children the Department of Surgery and the Psychiatry Gender Identity Program. The program operates a unique ambulatory multi-disciplinary clinic that includes primary care endocrinology gender affirming consultative care embedded social work services and medication management all delivered in a safe and supportive clinical environment.


While this role is primarily based at MGH in THP as we continue to evolve into a multidisciplinary integrated gender-affirming care program the role may ultimately support patients across MGH and BWH including disciplines such as general medicine/primary care surgery endocrinology and psychiatry.



Job Summary

The Clinical Social Worker directly reports to the Nursing Manager and the Medical Director of THP and indirectly to the MGH Department of Social Work. The Clinical Social Worker also works collaboratively with existing social work staff in THP.

As a Clinical Social Worker you will work closely with the clinical team to assess patient needs provide short-term counseling based on assessed needs and connect individuals with essential resources. Youll play a key role in enhancing patient well-being and improving care coordination and community connections.


Clinical Practice

Conducts diagnostic evaluations and assessments of patients and families with social emotional interpersonal environmental and/or psychiatric issues
Formulates diagnostic disposition and treatment plans
Conduct intake of pediatric gender affirming consult care patients alongside MD

Meet with new adult patients on an as-needed basis to orient to clinic and assess needs
Provide short-term counseling and crisis intervention on a limited basis as needed
Advocate for patients helping them navigate healthcare systems and community resources
Offer health education and promote self-management strategies
Collaborate with physicians APPs nurses and behavioral health specialists to enhance patient care
Takes responsibility for own administrative duties including timely and appropriate documentation in patient medical records timely and accurate daily reporting of activities and Hospitals scheduling systems and accurate reporting of time worked
Provides clinical documentation including psychosocial assessment progress notes and billing compliance (if appropriate)
Attends and participates in staff meetings and interdisciplinary meetings/rounds

Community Engagement
Attend and when appropriate help organize/facilitate meetings with existing community partners (e.g. Trans Community of New England Boston Urban Pride Trans Resistance Mass Trans Political Coalition New England Gender Care Mayors Office)
Facilitate new partnerships where appropriate to build and diversify client base
Ensure community representation on patient advisory committees and around research/educational oversight
Co-represent THP in national coalition meetings around sociopolitical climate safety and more



Qualifications

Qualifications

  • Masters of Social Work (MSW) from an accredited program required.

  • Current professional licensure in Massachusetts (LCSW required LICSW preferred.)

  • Minimum of 1 year post-licensure experience required; 3 years post-licensure experience preferred.

  • Experience with transgender and gender diverse patients preferred.

  • Demonstrates personal qualities of leadership and knowledge of health care system.

  • Strong assessment and treatment skills.

  • Good organizational skills.

  • Demonstrates the ability to communicate effective orally and in writing.

  • Strong interpersonal skills enabling effective team collaboration.

  • Interest in working on a dynamic inter-professional team.

  • Flexibility to evolve particulars of the role to meet the changing needs of the patient population served.

Skills/Abilities/Competencies

  • Essential to the role are abilities to ensure that the clinical environment is one that welcomes a diversity of patients and staff across the gender spectrum. This includes addressing all individuals with chosen name and pronouns and avoiding gendered honorifics or assumptions about gender based on physical appearance vocal tone or other features. In addition job candidates will have:

  • In depth knowledge of mental health diagnoses procedures and treatments related medications acuity versus chronicity effect on normal growth and development

  • Ability to provide mental health evaluations and assessments develop treatment plans that are behaviorally specific short-term and long- term treatment with individuals families couples and/or groups

  • Crisis intervention/treatment/management skills

  • Demonstrated competency in age specific behaviors; cultural and linguistic issues and developmental stages

  • Ability to assess and recognize diversity of clinical and resource needs across transgender and gender diverse populations

  • Knowledge of community agencies resources. Ability to advocate/negotiate systems for/with clients

  • Documents in medical record according to departmental policies and procedures

  • Maintains patient/family confidentiality
