Case Manager MyLife (12 month contract)

Location: Pakenham (moving to Lilydale Office in November 2025).

Full-time Fixed Term (12 months).

Salary: $100783 $105349 super salary packaging.

Anchor is seeking a skilled and compassionate Case Manager to join our MyLife team delivering person-centred trauma-informed support to children young people and families.

This is a unique opportunity to work in a multi-disciplinary team delivering Targeted Care Packages flexible individualised support to help young people transition out of (or avoid entering) residential care. Initially based in Pakenham the role will relocate to Lilydale in November 2025.

Your role as Case Manager

Youll work closely with Youth Workers and other key supports to:

  • Deliver holistic case management tailored to the needs and goals of each young person.
  • Partner with young people families and professionals in their care team.
  • Embrace evidence-based practice grounded in developmental trauma positive psychology and self-determination theory.
  • Promote safety stability connection and personal growth for every client.
  • Record and evaluate outcomes using Anchors program tools and frameworks.

About You

Youre a motivated practitioner who brings:

  • A tertiary qualification in Social Work Psychology Youth Work or related discipline (final-year students may be considered).
  • A solid understanding of the Children Youth and Families Act 2005 and trauma-informed practice.
  • Experience in case management within child youth or family services.
  • An ability to work with young people from diverse and complex backgrounds including those in the statutory care system.
  • Strong interpersonal advocacy and organisational skills.
  • A collaborative mindset and a passion for creating positive change.
