Position: Case Manager 2 Intake Specialist
Program: REACH
Location: New Haven
Salary: $21.00/hour
Schedule: Full time 40 hours 1st shift Monday-Friday
Program Summary:
Providing short-term supportive housing and case management services for adults re-entering the community after incarceration to assist them in obtaining employment identifying long-term housing and maintaining a substance and crime free lifestyle.
Position Summary:
This position is responsible for leading shift operations during designated periods providing case management services to a specified case load of residents supportive counseling and coordinating resident services. The Case Manager 2 functions as a team member of the program and as a member of designated resident treatment team. This position is responsible for carrying a caseload for implementing and facilitating resident activities/interventions and for providing program shift coverage and leadership.
Working within Adult Services the Case Manager 2 works with Community Justice program staff to help to rehabilitate educate and successfully guide clients from incarceration back into the community.
Requirements:
- Bachelors Degree
- 2 years of related experience in the direct service and case management field
- Valid Connecticut drivers license
- Regular and predictable attendance is required
Orientation:
If selected for employment with The Connection newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
