QUALIFICATIONS:
- Must meet certification requirements as specified in Bulletin 746.
- Must have a valid license to practice occupational therapy in Louisiana in compliance with R.S. 37:3001-3014 as administered by the Board of Medical Examiners.
APPLICANTS MUST PROVIDE COPY OF ANCILLARY CERTIFICATE AND LAST TWO (2) COMPLETE EVALUATIONS.
