Purpose: Under the direction of a Team Lead the Wraparound Facilitator is responsible for providing ongoing intensive case management services through the Wraparound facilitation as evidenced by effective assessments and interventions with clients.
Provides training to the client and natural supports that addresses the serious emotional disturbance and symptom-related problems that interfere with the clients functioning provides opportunities to acquire and improve skills needed to function as appropriately and independently as possible in the community and facilitates the clients community integration and increases his or her community tenure. This service includes treatment planning to facilitate resiliency.
Qualifications:
Wraparound Facilitator A staff member who is credentialed as a QMHP who has at least a bachelors degree demonstrated and documented competency in the work to be performed and has:
- A minimum of a bachelors degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology social work medicine nursing rehabilitation counseling sociology human growth and development physician assistant gerontology special education educational psychology early childhood education or early childhood intervention.
- A Registered nurse (RN) licensed as a registered nurse by the Texas Board of Nursing in accordance with Texas Occupations Code Chapter 301
Must be certified in the Wraparound Facilitator Training
General Responsibilities: The Wraparound Facilitator is responsible for providing medical necessary services to the client in accordance with the treatment plan that is designed to meet the mental health educational vocational residential financial social and other needs of the client.
Follow Texas Department of State Health Services Resilience and Recovery Guidelines by completing reassessments: treatment plans CANS/ANSA and ensure that annual assessments are completed on caseload
Serve as an intricate member of the LESTX team to ensure continuity of care and supports the services 24 hours a day seven days per week as necessary
- Provides structured face-to-face scheduled sessions to client based on the treatment plans
- Provide services in the home school office and community setting
- Documentation completed within 72 hours of service
- Ability to network with schools juvenile probation and other social service agencies to facilitate services for youth.
- Responsible for coordination planning and implementation of the wraparound plan as the individual plan of care.
- Ability to communicate with youth and their families to promote collaboration when creating wraparound plans.
- Performs clinical assessments to determine service needs and progress with consumer treatment goals.
- Participates in formulation of the consumer wraparound plans and assures wraparound plan is written and authorized with an LPHA signature for consumers on caseload.
- Ensures all services provided meet eligibility criteria and are authorized in accordance with agency and regulatory guidelines.
- Provides skills training services that address the severe and persistent mental illness and symptom related problems that interfere with the individuals functioning.
- Provides interventions in response to crisis to reduce symptoms.
- Performs activities to assist consumers in gaining and coordinating access to necessary care and services based on individual needs.
- Provides training/education on the nature of mental illness the importance of medications and other medication related information
- Demonstrate professional work ethics as evidenced by overall dependability conscientiousness and timeliness.
- Ensure all necessary licenses and/or certifications remain current
- Other duties as assigned
