of Job Duties:The Pharmacist PRN for the clinical pharmacy position provides clinical pharmacy expertise on an as-needed basis including but not limited to after-hours and back-up support on weekends and/or observed holidays. The pharmacist will work under minimal supervision and will use their knowledge skills and clinical judgment through reviewing orders to ensure safe and effective medication therapy. The pharmacist will monitor patient responses to drug therapy including laboratory values and clinical indicators to identify and resolve medication-related problems. The pharmacist will work closely with physicians nurses and other healthcare professionals to coordinate care and optimize medication management. The pharmacist will document in the electronic medical record as appropriate.


Qualifications:

Minimum Qualifications:

  • Bachelors degree in Pharmacy plus 1 year of experience working as a pharmacist (or pharmacy intern) in a retail or hospital setting

  • Certification as a licensed Registered Pharmacist by the State of Arkansas or eligible for licensure by the State of Arkansas

Preferred Qualifications:

  • Graduate of an ACPE-accredited School of Pharmacy with a Pharm.D.

  • 1 year of applicable pharmacy experience


Additional Information:

Responsibilities:

  • Reviews and interprets prescribers orders and detects appropriateness of therapy.

  • Prepares and dispenses medications.

  • Prepares sterile admixtures using proper aseptic technique.

  • Prepares chemotherapy according to departmental policies and procedures.

  • Answers client questions regarding prescription renewal dosage and side effects.

  • Serves as a primary or co-preceptor for rotational students.

  • Provides formal in-depth training for students and new staff.

  • Assists with the development of educational materials and in-service training for staff development.

  • Directs advises and instructs staff in the assigned area in all job duties and functions.

  • Assists leadership in all aspects of pharmacy operations in the area of assigned responsibility.

  • Serves as a liaison between the pharmacy service line and the assigned practice area.

  • Promotes a safe working environment by adhering to the UAMS patient safety plan.

  • Ensures equipment and facilities are used calibrated and maintained properly.

  • Initiate adjust and monitor pharmacokinetically-dosed medications (e.g. vancomycin aminoglycosides warfarin).

  • Utilize hospital protocols and evidence-based guidelines to recommend and adjust doses.

  • Interpret serum drug concentrations and lab data to optimize therapy.

  • Communicate recommendations effectively to providers and document interventions appropriately.

  • Fulfills other duties as assigned.
