We are a care management organization that provides triage evaluation and structured follow-up care for senior patients throughout the states. Our mission is to support medical practices in improving outcomes reducing readmission and promoting high-quality remote care for the aging population.
Position Summary
We are hiring Remote Nurse Practitioners to perform telehealth-based clinical assessments and follow-ups for geriatric patients. This non-prescribing role focuses on patient evaluation and ongoing care coordination.
New graduates are welcome to apply and while no formal training is provided clinical supervision will be available to support professional growth and ensure high-quality patient care.
Conduct remote evaluations and follow-up assessments via secure telehealth platforms
Perform triage and assess symptoms in non-emergent cases
Follow up within 24 72 hours of discharge or provider visits
Document all patient interactions in a HIPAA-compliant EHR
Collaborate with supervising clinicians and escalate care when appropriate
Requirements
- Active New York State Nurse Practitioner license (Required)
- Comfortable working remotely and independently within a supervised structure
- Excellent verbal and written communication skills
- Strong organizational and time management abilities
- Experience or interest in senior care primary care or chronic care management preferred
