The Cancer Center at Beth Israel Deaconess Medical Center provides state-of-the-art cancer care through personalized treatment plans clinical excellence and compassionate support.

The Department of Pharmacy is seeking a highly motivated Clinical Pharmacy Specialist to join our team and support the expanding inpatient hematology/oncology and cellular therapy services. This role is an integral part of a multidisciplinary care team responsible for designing implementing and monitoring individualized care plans to ensure safe effective and cost effective treatment.

Key Responsibilities:
Actively participate in multidisciplinary patient care rounds
Collaborate with physicians nurses and other healthcare professionals
Precept pharmacy students and residents
Provide patient education and discharge counseling
Contribute to the development and implementation of clinical policies protocols and treatment guidelines
Support education quality improvement initiatives and research aligned with the Cancer Centers mission of delivering exceptional care and advancing medical discovery

Job Description:

1. Practices in accordance with the state and federal laws rules and regulations including but not limited to Massachusetts Board of Registration in Pharmacy Department of Public Health the Food and DrugAdministration and the Drug Enforcement Administration. Maintains certification and credentials in accordance with the Board of Registration and adheres to institutional policies and procedures.

2. Reviews and verifies medication orders with consideration including but not limited to the following patient specific parameters: diagnosis weight/body surface area age allergies organ function laboratory values medication duplication drug interactions. Clarifies orders deemed to be inappropriate incomplete or otherwise unclear with the prescriber in a timely manner.

3. Documents and communicates pertinent information and discussion with the healthcare team and pharmacy staff to ensure continuity of care.

4. Provides drug information to physicians nurses and other health care professionals.

5. Checks and verifies sterile and non-sterile medications.

6. Supervises the activities of students technicians and residents within the pharmacist area of practice.

7. Promotes a culture of patient safety by submitting medication related events and near misses.

8. Completes daily pharmacy surveillance tool monitoring including follow-up and documentation as well as other daily patient care initiatives and relevant reports.

9. Works collaboratively with all pharmacists pharmacy technicians and pharmacy personnel. Upholds a culture of respect and teamwork across the Pharmacy Department.

10. Precepts pharmacy students resident rotations and resident-led quality improvement projects. May serve as Residency Program Coordinator or Residency Program Director

11. Regularly participates in department initiatives including but not limited to supporting formulary stewardship presenting new medication requests at Pharmacy & Therapeutics Committee creating and maintaining policies and procedures conducting assessments of workflows and/or developing departmental tools and resources.

12. Supports a specific core area of practice through interdisciplinary education ongoing specialty training and as a resource for Pharmacy staff.

13. Updates clinical guidelines and presents hospital wide educational initiatives.

14. Serves on hospital interdisciplinary committees or workgroups to improve process and quality outcomes.

15. Supports IRB-approved research activities medication use evaluations or cost-savings initiatives.

16. Other duties as assigned

Required Qualifications:

1. PGY 1 1-3 years OR PYG2 OR 3-5 years of experience in clinical specialty area
