We are seeking a highly skilled and empathetic Palliative Specialist (PA or NP) to join our this role you will provide palliative care to patients focusing on symptom management pain relief and improving the quality of life for individuals with serious health conditions. This position is a mobilein-person patient care role with the opportunity to collaborate closely with a multidisciplinary team of healthcare professionals.
Provide comprehensive palliative care consultations to patients with chronic or serious illness.
Develop and implement individualized care plans focusing on symptom management pain relief and emotional support.
Collaborate with physicians nurses social workers and other healthcare providers to ensure comprehensive team-based care.
Educate patients and their families about illness progression treatment options and goals of care.
Monitor patient outcomes and adjust care plans as necessary.
Participate in interdisciplinary team meetings and case reviews.
Current valid certification as a Physician Assistant (PA) or Nurse Practitioner (NP) with specialization in palliative care geriatrics or related field.
Certification such as Advanced Certified Hospice and Palliative Nurse (ACHPN) is preferred.
Minimum of 13 years of experience in palliative care hospice or chronic disease management.
Strong communication skills with the ability to provide compassionate care and counseling to patients and families.
Ability to work independently manage a diverse caseload and coordinate care in a remote setting.
Experience in telehealth is a plus.
Current Nevada licensure (or eligibility to obtain).
Must have a valid drivers license reliable personal vehicle and active vehicle insurance.
