Make an Impact as an Outreach and Referral Coordinator!

The NewPath Child & Family Solutions Outreach and Referral Coordinator is responsible for maintaining internal and external relationships to ensure timely and clinically appropriate placements into NewPath Child & Family Solutions. The referral coordinator is well versed on the intake parameters for each service line within the agency. This position is the main point of contact for all referrals and expected to review collaborate with program leadership and respond in a timely manner.

What youll be doing:

  • Process referrals received by program.
  • Prioritize referrals to promptly get care for clients.
  • Cultivate and manage referral partner relationships allowing for streamlined communication.
  • Support clients throughout the referral process.
  • Provide ongoing communication and support to ensure successful outcomes.
  • Collaborate with internal teams and external stakeholders.
  • Optimize and grow client referrals.
  • Provide follow-up support to ensure successful referrals.
  • Identify new potential partners based on referral trends and patterns.

Qualifications:

  • Highschool Diploma/GED required; Associates/Bachelors in relevant field preferred.
  • 1-3 years experience in Administrative Healthcare or Community Health setting.
  • Excellent customer service skills to interact with clients families and internal/external stakeholders.
  • Strong communication and engagement skills.
  • Strong organizational and time management abilities.
  • Ability to interact in a positive manner with families and children.
  • Excellent computer skills including knowledge of Microsoft Office.
  • Must have a valid drivers license and proof of current auto insurance with no more than 5 points on your MVR.
  • Must be able to pass a criminal background check.
