This position will service the Kentwood Wyoming Byron Center Waylon and Shelbyville areas
Under the direct supervision of the Nursing Manager and in collaboration with the interdisciplinary team the Registered Nurse uses clinical judgment skills to access and implement the treatment plan for patients. The RN assesses monitors detects plans and takes appropriate action to prevent physiological complications associated with specific health problems and/or medical treatment.
Communicates effectively with staff residents and families. Assesses develops and maintains an individualized patient plan of care with the patient.
As needed specified by CH entity: Performs Medication Administration Pass according to regulatory guidelines. Informs MDS coordinator of significant resident/patient changes needed for completion of the MDS.
Communicates with other members of the health care team in order to ensure continuity of care and coordination of services. Acts as a patient/family advocate.
Evaluates patients and significant others educational needs and develops and implements appropriate teaching strategies or makes appropriate referrals.
Observes and reports the patients condition needs and outcomes of intervention to the physician and other team members. Identify problems and emergency situations and initiate immediate life saving measures in the absence of a physician.
As needed specified by CH entity: Teaches and supervises the care provided by CNA LPN or other nursing service personnel according to regulatory and agency guidelines.
Implements the plan of care by providing nursing services including teaching treatments diagnostics and preventive procedures regularly evaluating the patients progress toward established goals recommending revisions to the plan as necessary and planning for termination of service or discharge. Assumes a leadership role and delegates appropriately to support staff to facilitate safe quality patient care in a cost-effective manner.
Participates in ongoing process improvement activities through the application of clinical paths QA processes and other related activities. Ensures that processes and services are continuously monitored for quality cost effectiveness and efficiency.
Required
Associates Degree Nursing
2 years of relevant experience
LIC-Registered Nurse (RN) - STATEMI State of Michigan
LIC-Drivers License - STATEMI State of Michigan
CRT-Basic Life Support (BLS) - AHA American Heart Association Or
CRT-Basic Life Support (BLS) - ARC American Red Cross
Preferred
Bachelors Degree Nursing
1 year of relevant experience of acute care emergency or rehab facility experience for all of the Home Care business lines
