Job Description
- Establish professional relationships with Lake County resources for social determinants of health.
- Respond to requests for assistance with social determinants of health from Vernon Township residents
- Coordinate and facilitate community education events.
- Holiday gift drive for youth who may not receive holiday gifts
- Assist residents in the completion of Medicaid and Managed Care Organization applications.
- Connect Vernon Township residents to housing resources and eviction prevention support.
- Connect Vernon Township residents and assist them in the process of applying for support from utility companies.
- Provide connection to food pantries and other resources for food and basic needs.
- Develop system to track program participants and the services they receive.
- Link individuals to appropriate counseling services.
- Completes other tasks which may be assigned to support agencys mission
- Bachelors degree in a human service-related field (required)
- Two years experience in community outreach
- Bilingual in Russian (required)
- Knowledge of community-based services and partnerships
- Strong organization skills
- Valid drivers license and proof of insurance (Required)
