Were hiring for a full-time Family Nurse Practitionerto join our Perry Hall Facility.

What Youll Be Doing

RESPONSIBILITIES: As an FNP with the new Perry Hall you would be providing strength-focused evidence-based and culturally sensitive treatment. Creating and sustaining positive compassionate interactions with patients and providing high quality care will be central to the work you do. You would conduct assessments manage cases and provide supportive therapy with utmost kindness and care. Your attention to detail and problem-solving skills will be important to your responsibilities related to evaluating and managing medications. Furthermore youd have the distinct opportunity to work closely with a multidisciplinary team of Health Solutions professionals to serve our community.

Specifically the FNP responsibilities under the administration of the Director of Medical Services will:

Administrative duties

  • Maintain accurate and current records with 100 percent compliance with Health Solutions standards
  • Use electronic health records and electronic prescribing accurately and facilely to promote patient care

Clinical duties

  • Conduct assessments and engage with patients with kindness and a focus on their needs
  • Prepare and administer prescribed medications injections and vaccinations
  • Facilitate timely and accurate medication prescriptions and refills
  • Maintain medications securely
  • Care for wounds
  • Maintain and organize medication records
  • Review laboratory results with the treatment team and patients
  • Apply best practices in medical care and medication assisted treatment
  • Monitor chronic disease markers and review them with the treatment team and patients
  • Use developmental milestones in assessing and caring for patients
  • Offer psychoeducation to patients and their family about topics relevant to high quality care (e.g. disease processes sleep hygiene nutrition behavior modification)
  • Implement preventive medical interventions in collaboration with patients and healthcare specialists
  • Evaluate behavioral health assessments and makes referral to behavioral health professionals
  • Witness urine toxicology screenings and performs oral drug testing
  • Maintain productivity

Care integration and collaboration

  • Participate in and effectively collaborates with our multidisciplinary teams of professionals to promote the best quality care
  • Participate as a valuable team member in meetings and consultations with other members of the team
  • Perform other job-related responsibilities as assigned

Physical requirements include the following:

  • Frequently stays stationary for prolonged periods of time.
  • Frequently moves within and between facilities.
  • Frequently exposed to stressful or emergent situations.
  • Frequently operates a computer and other office equipment to record health data.
  • Constantly communicates with clients and others.
  • Occasionally moves equipment and/or materials up to 50 pounds.
  • Occasional exposure to communicable diseases and bodily fluids

Qualifications

What were looking ForThe Must-Haves

  • Current license as a Family Nurse Practitioner in Colorado
  • Graduate degree in nursing from an accredited academic institution
  • A minimum of 5 years of experienceas an RN
  • 3 years of experience as FNP
  • Computer proficiency

  • Experience caring for individuals with addictions and severe and persistent mental illness
  • Track record of working successfully as a part of a multidisciplinary team
  • Experience in managed care environments
  • Current Colorado drivers license; Access to reliable transportation during work hours
  • Spanish language skills
