Your part in our State-wide Palliative Care team
BlueCares Palliative Care services operates at a state-wide level and we are expanding the capacity and capability of our team allowing us to deliver more services across Queensland particularly in regional & remote locations. Reporting to the Palliative Care Manger the role will work in conjunction with the relevant community neighbourhood and is an essential role for the distribution of seamless palliative care specific services ensuring the delivery of high quality bespoke in-home care to clients and carers through current and future Government funding models. Additionally you work in a variety of areas including but not limited to:
Accept internal and external referrals for Palliative Care services and engage with client in a timely manner.
Responsible for the scheduling of all Palliative Care services.
Provide professional first point of contact/concierge services to clients and carers ensuring clients individual needs and preferences are adhered to.
Respond to client and carer enquiries and escalate to relevant neighbourhood or allied health professionals where required.
Maintain effective communication and problem-solving skills ensuring effective and rapid referral to other health care professionals including Palliative Care consultants nursing and allied health teams.
Engage effectively with community neighbourhoods and or Specialised Services State-wide contacts in the identification of additional client needs
What makes a difference for us
We provide ongoing training and support to the successful individual around our systems program and engagement requirements. You will be supported by leaders directly within the Palliative Care Program to provide holistic support in the role. If you possess the following skills it will certainly give you a head start in the role.
Certificate III qualification or higher in a Health Administration Community Service or other relevant discipline or equivalent community sector experience.
Demonstrated experience in connecting teams customer service and problem solving within a multidisciplinary team.
Demonstrated ability to liaise with clinical staff to provide co-ordinated care and exercise professional judgment within prescribed areas.
Demonstrated ability to understand the complex needs of customers and to ensure they receive the services appropriate for their family circumstances.
Demonstrated experience in dealing with complex funding models and compliance with stated guidelines and contractual obligations within as set by the relevant Government Departments.
Demonstrated high-level interpersonal and written communication skills including intermediate computer skills (e.g. Microsoft Office Suite).
Great time-management skills with an ability to prioritise your workload in a fast-paced environment.
