Through our interdisciplinary model of telehealth physicians in-home paramedics and RN care managers myLaurel is redefining where and how acute care is delivered.
Were hiring a Clinical Liaison (RN) to support the launch and growth of a new hospital-partnered service line in Philadelphia. Youll act as the face of myLaurel inside the hospitalbuilding trust identifying eligible patients and promoting our home-based acute care programs.
This is a hybrid clinical-strategic role perfect for a nurse who excels in care coordination patient education and stakeholder engagement.
Hospital Engagement Build strong relationships with care teams physicians and hospital leadership
Referral Management Identify eligible patients and guide them into myLaurels acute care programs
Patient Education Clearly explain services and care plans to patients and caregivers
Operational Integration Seamlessly embed myLaurel into hospital discharge and care workflows
Strategic Growth Collaborate with internal teams to drive program adoption
Track Results Monitor key performance metrics including referrals patient satisfaction and outcomes
Cross-Team Collaboration Coordinate with myLaurels internal care and operations teams
Attend hospital huddles and rounds
Conduct chart reviews and referral assessments
Deliver program education to patients caregivers and staff
Track referral data and outcomes
Serve as a clinical ambassador for home-based acute-level care
Active unrestricted RN license
5 years of clinical experience (hospital inpatient or ER preferred)
Expertise in care transitions discharge planning and readmission prevention
Strong communication and relationship-building skills
Proficient in using EHRs and navigating hospital systems
BSN or related degree preferred
