Promote the timely flow of cancer patients through the healthcare continuum of care with heavy emphasis on identifying gaps in care and supporting the patient from the time of diagnosis to the end of treatment. Execute multiple functions relating to direct patient care patient education and advocacy care coordination and psychosocial support. C. PREREQUISITE SKILLS
Embodies all of MarinHealths C.A.R.E.S. standards. Intermediate to advanced computer skills. Must have working knowledge of Microsoft Office applications and electronic medical record systems. Exceptional inter-personal skills required for connecting with patients as well as successfully interacting with colleagues internal physicians and staff referring physicians and community organizations. Ability to work independently on prioritizing job responsibilities and multiple tasks around deadlines troubleshooting and resolving issues and following procedures without direct supervision. Highly attentive to detail and follow through. Proven project management skills. Requires up to 25% travel to offsite outreach programs and events. Excitement for working in entrepreneurial fast-paced environment. Must be available to work occasional nights and weekend. Bi-lingual (English & Spanish) preferred.
V. LICENSE/REGISTRATION/CERTIFICATION:
A. Current California Associate Clinical Social Worker (ASW) registration required.
B. Current California Clinical Social Worker license (LCSW) preferred.
C. Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

The protected health information a person in this position can access includes demographics date of service insurance/billing medical record summary information and all other information that may be contained in patient records. This position requires patient health information to perform the functions outlined as part of this position description.
VIII. DUTIES AND RESPONSIBILITIES:
A. ESSENTIAL (not modifiable)
Navigate:
Navigate recently diagnosed cancer patients to appropriate health care services in the MarinHealth system and through the ins-and-outs of their illness recovery and survivorship. Coordinate with other health care providers to ensure seamless cancer care (e.g. pathologists radiologists oncology nurses radiation therapists dietitians social workers integrative practitioners financial counselors and administrative staff) Help patients communicate successfully with their physicians and other health care providers. Actively participate as patient liaison to tumor board and other multidisciplinary conferences.
Support:
Support decision-making for patient by providing educational resources and escalating concerns and questions to physician. Help patients anticipate and solve problems; prioritize issues; and eliminate barriers to care (such as financial/insurance emotional transportation child-care and/or work-related barriers) by referring to appropriate individuals or services.
Integrate:
Follow patients and family members across department lines serving as link and bridge between relevant MHMC programs from the point at which they are diagnosed with cancer through treatment. Integrate a full-range of complementary therapies that can support patients treatment recovery and survivorship. Connect patients with local and national community resources support and reputable health care information. Work with other cancer navigators to ensure cross-coverage and collaboration of ideas.
Administrative:
Identify improvement opportunities and drive execution of these ideas. Pro-actively develop and maintain collaborative relationships with appropriate physicians and organizations (i.e. local primary care and specialty care practices). Maintain standardized patient education tools and formulate new standards and policies to improve patient navigation and reduce costs. Set and uphold program performance metrics and monitoring; track patient satisfaction. Research and identify current best practices for cancer navigation.

The compensation for this role listed on this posting is in compliance with applicable law. The selected candidates compensation will be determined based on the individuals skills experience internal/market equity factors and qualifications. This posted minimum and maximum range represents the minimum and maximum of what we reasonably expect to compensate for the position. Furthermore all compensation decisions are ultimately determined in accordance with our compensation philosophy. Compensation for positions covered by collective bargaining agreements are governed by the agreements in the aforementioned document.
