Your Opportunity
The AOD Continuing Coordinated Care (CCC) Service in Far West NSW provides care coordination and wraparound services for people living with concerns related to their substance use who are receiving community-based treatment or are looking to explore residential treatment options while also experiencing a high level of need.
You will engage in providing case management and case co-ordination to individuals with Alcohol and other drug support needs including harm reduction in relation to substance use housing parenting and family support income and employment.
You will be required to be creative in providing harm minimisation and relapse prevention strategies to support participants to address issues and challenges that are faced throughout recovery to achieve their desired outcomes.
