Job Description
A healthcare provider is looking to hire a compassionate and experienced Nurse Practitioner (NP) to deliver quality care to patients in a home-based setting across Brooklyn. The ideal candidate is highly organized patient-focused and able to work independently. This is a full-time weekday position (no weekends) with reimbursement for travel expenses.
Responsibilities:
- Conduct patient visits in-home providing comprehensive assessments and ongoing care
- Collaborate with physicians and other care team members to coordinate treatment plans
- Document visits and patient updates accurately and in a timely manner
- Monitor patient progress and adjust care plans as needed
- Maintain strong communication with patients and families
Qualifications:
- Licensed Nurse Practitioner (NP) in the state of NY
- At least 1 year of clinical experience preferred
- Must own a car and be comfortable driving in Brooklyn
- Strong interpersonal and time-management skills
- Able to work independently and manage a mobile schedule
Location: Brooklyn NY Williamsburg (Travel Required)
Type: Full-time (Weekdays Only)
Salary: $155000$190000 BOE
