Description

Medical Records Clerk

Summary

The Medical Records Clerk is responsible for the maintenance and preservation of the medical record paper and electronic including retrieval processing content organization and storage. The Medical Records Clerk will take the utmost precautions to assure that medical record is protected from loss or tampering. The Medical Records Clerk will be accountable for performing clerical and administrative functions that support the Medical Records Department. The ideal candidate will possess the ability to meet strict deadlines work in a high volume fast paced multi-tasking environment without compromising accurate patient data. In addition the candidate will be expected to demonstrate excellent phone etiquette customer service skills and have advanced computer skills. This position is responsible for all aspects of the medical record.

Responsibilities and Duties

  • Has a thorough working knowledge of HIPAA Privacy Practices and Release of Information policies and procedures and can assist patients and staff relative to Department responsibilities regarding authorizations patient access request for amendments accounting of disclosures and minimum necessary standards
  • Has an excellent understanding of Medical Records processes and helps maintain an accurate and efficient filing and EHR System
  • Demonstrates knowledge and understanding of the rules of confidentiality chart analysis alphabetical and terminal digit filing scanning into the Electronic Health Record (EHR) and all policies and procedures associated with the department
  • Performs break-down analysis assembly and scanning of the medical record
  • Accurately sorts and routes medical reports both internal and external to the appropriate provider before the patient is seen
  • Responds to patient medical record requests and releases records per HIPAA directives
  • Answers phone calls with excellent telephone and customer service etiquette
  • Maintains an organized medical record storage area
  • Assists with purging medical records per retention directives
  • Assists with inactivating patients in the EHR
  • Maintains productivity standards for scanning for a minimum of 200 scans when volume is sufficient
  • Performs break-down analysis assembly and scanning of medical records
  • Performs other duties as assigned demonstrating flexibility and willingness to do extra work as needed on an intermittent basis.
  • Follows all NRHC policies and procedures

Qualifications

Education and Experience:

  • High School Diploma or equivalent
  • At least 1 year of medical records experience in a clinic or hospital setting is preferred
  • Maintains strict confidentiality. Does not disclose confidential information regarding patients co-workers or the organization to unauthorized individuals in or out of the organization.
  • Is responsive to supervisory feedback utilizing constructive criticism in a positive manner
  • Team oriented individual who shows initiative and demonstrates flexibility
  • Task-oriented and self-starter
  • Ability to maintain composure when faced with difficult situations
  • Must be able to handle confidential situations with tact
  • Maintains open communication and observes chain of command
  • Ability to work with computers scanning devices electronic devices telephones fax machines
  • Ability to understand and follow oral and written directions
  • Ability to communicate effectively gain credibility and develop productive relationships with management staff members and other customers
  • HIPAA Knowledge
  • Knowledge of Medical Terminology and Anatomy and Physiology is desired but not required
  • Attends staff meetings as needed
  • Possesses a valid Drivers License and liability insurance
  • Observes the dress code; appearance and grooming are appropriate

Physical demands/essential functions: Physical demands/essential functions: Frequent: sitting standing walking working on computer 8 hours daily lift/carry objects up to 24lbs repetitive motion of hands & wrists close eye work color differentiation independent problem solving. Constant: listening speaking in person or on the phone reading writing working with the public and staff.

Employee provides services associated to the Northeastern Rural Health Clinics its participating physicians and clinicians which is a covered entity under the HIPAA rule. In the scope of performing functions including but not limited to management administrative financial legal and operational support services I may have access to Protected Health Information (PHI) which is information whether oral written electronic visual pictorial physical or any other form that relates to an individuals past present or future physical or mental health status condition treatment service products purchased or provision of health care and which reveals the identity of the individual whose health care is the subject of the information or where there is reasonable basis to believe such information could be utilized to reveal the identity of that individual.
