Position Summary:
The Licensed Practical Nurse (LPN) in Home Health provides direct nursing care to patients in their homes under the direction and supervision of a Registered Nurse (RN). The LPN is responsible for delivering compassionate safe and effective care that is aligned with the patients individualized plan of care while recognizing the unique needs of patients across the lifespan. The LPN demonstrates clinical competency sound judgment and a commitment to promoting health comfort and safety in the home setting.
Deliver skilled nursing care and treatments as ordered by the physician and delegated by the RN in compliance with home health regulations and standards.
Collect and assess data reflective of the patients status and contribute to the development and revision of care plans.
Administer medications and treatments safely and accurately while monitoring for side effects and effectiveness.
Monitor patient progress identify changes in condition and promptly report to the supervising RN or physician.
Document all care and interventions thoroughly accurately and timely in the electronic medical record.
Educate patients and family members on disease processes medications treatments and home care techniques to support self-care and prevent complications.
Provide age-appropriate and culturally competent care demonstrating awareness of population-specific standards.
Maintain effective and professional communication with patients families physicians and interdisciplinary team members.
Ensure patient safety in the home environment by recognizing hazards and implementing preventive measures.
Participate in team meetings case conferences and performance improvement activities as required.
Maintain patient confidentiality and adhere to ethical and legal standards in all aspects of practice.
Current and unrestricted Licensed Practical Nurse (LPN) license in the state of MN.
Minimum of one year clinical nursing experience home health experience preferred.
Current CPR certification.
Valid drivers license and reliable transportation.
Proficiency in electronic documentation and technology use.
Solid knowledge of nursing principles medication administration and care coordination.
Familiarity with chronic disease management wound care and other common home health interventions.
Strong observational and assessment skills.
Ability to establish trust and rapport with patients and families in a home environment.
Effective verbal and written communication skills.
Adaptability to changing patient needs schedules and home environments.
Commitment to professional development and upholding standards of safety quality and compliance.
Works in patients homes in various environments and conditions.
Regular travel within assigned service area.
Requires physical activity including walking standing bending and lifting.

Qualifications
Graduate from an accredited practical nursing program as a Licensed Practical Nurse. If position requires IV therapy/management skills the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.Sanford is an EEO/AA Employer M/F/Disability/Vet.
