Consultation Liaison Nurse - HIV Mental Health

About the role

The CL Nurse HIV Mental Health coordinates the VHMHS working in collaboration with the other team members. Professionally the position will be supported through the Hospital and Emergency Service framework and participate in the clinical and professional forums within nursing and consultation liaison psychiatry.

Internally the position will interact with hospital-based services specialist mental health teams and externally to other health care agencies involved in the care of People Living with HIV (PLHIV).

The clinical aspects of the role include the following in collaboration with the psychiatric registrar or Psychiatrist.

Provision of nursing assessment and care planning for specific clients within the acute health and psychiatric services of the Alfred.

Engage in discharge planning for patients with mental health diagnoses.

The incumbent will also perform as part of the core responsibilities

Triage for outpatient psychiatric assessment incorporating brief telephone risk assessment and interim management advice based on communicated concerns.

Provide secondary consultation and offer advice to staff within the Victorian HIV Service community care providers and community agencies.

Within the HIV Outward Program provide care coordination for patients with mental health diagnoses that are beyond the scope of HIV nurses and social work team including monitoring/risk assessments for individuals with complex psychiatric and/or health issues living in the community. This will include at times undertaking community outreach assessments in collaboration with the HIV Outward Program.

Provide education to health professionals and community care providers on HIV and mental health.

About you

  • Current Registered as a Registered Nurse with Nursing and Midwifery Board of Australia via AHPRA.
  • At least 5 years experience as a mental health nurse working with a post graduate qualification in mental health nursing
  • Advanced clinical knowledge and skills in Consultation Liaison or HIV Nursing
  • Master qualification or working towards equivalent.
  • Strong Interpersonal and communication skills
  • Ability to work collaboratively with other services and departments across the hospital and community
  • Capacity to think critically
  • Ability and willingness to develop others
  • Commitment to quality and safety activities / research process and application to practice
  • Current and satisfactory Police Check
  • Current and satisfactory Working with Children Check
