JOB SUMMARY:
Under the general supervision of the Program Director/designee functions as part of an interdisciplinary team assisting assigned individuals in developing and maintaining viable living working and social situations in the community by obtaining needed medical social psychosocial educational vocational and other services. Ensure a smooth transition from foster care into Supported Housing. Staff will collaborate with external care coordinators assess consumer needs and discuss/advocate for needed services. Travels to/visits consumers apartments to provide counsel and assistance to help the consumers within their capabilities and interests in maintaining the greatest degree of independent living. The majority of duties are often performed independently under general supervision.

- Travels to/visits consumers residences to provide supportive services on a schedule established by the program.
- Assist the consumer to modify attitudes and patterns of behavior by increasing understanding of self and others. Teaching consumer skill development focuses on crisis management and reacting to emotional situations.
- Provide oversight on all high-risk clients linking them to treatment as well as guidance to the case manager on how to effectively help the clients manager crisis.
- Maintains up-to-date accurate individual case records on each assigned consumer and contributes to the development of measurable and objective service plans that maximize consumer rehabilitative abilities in accordance with regulatory guidelines.
- Carry a active case load of all high-risk clients as well as provide oversight on the additional client that are not deemed high risks
- Plans escorts and/or arrange social or recreational activities for and with consumers.
- Facilitate case conference with the remainder of the team to provide updates on high-risk clients.
- Promote recovery self-empowerment and hope in all client interactions

- Attends regularly scheduled clinical meetings staff meetings and supervision as well as in-service training and development activities.
- Provided one on one support/training on integrated into the community in addition to teaching them to develop skills that will help them adjust to the community.
- Secures information such as medical psychological and social factors contributing to the individuals situation and evaluates these and the individuals capabilities.
- Co-lead Family Support Groups with Director.
- Facilitate a variety of Lounge Groups such as GED tutoring Video Night Games and others.
- Organize Family Program Educational Open Houses.
- Assist families needing referrals and help with the Mental Health system.
- Respond to and follow up on Resource Line requests for information and referrals.
- Assist in coordinating monthly Ground Rounds events.
- Maintain family related duties as assigned.
- Perform other related duties as assigned.

QUALIFICATIONS AND EXPERIENCE:
Bachelors Degree in Human Services or related field plus two years experience in the mental health field. Experience working with individuals in independent living.
