Our Values
Pair Team is hiring a full-time Bilingual Community Health Worker (CHW) to support a high-need Medicaid population through whole-person relationship-based care. This role is a key part of Pair Teams Care Management Program which delivers coordinated medical behavioral and social services to individuals with complex health needs.
The CHW will engage members who may be navigating housing instability chronic health conditions or behavioral health challengeshelping them stay connected to care build trust in the system and meet their personal health goals.
This position primarily allows for remote work; however it includes regular in-person visits in the community across Clark County (Las Vegas). Typically 1 or 2 days per week will include visiting patients in the community at their homes and in various sites of care.
