Job Description

Job Summary

Supports and is responsible for incorporating into job performance the Frederick Health (FH) mission vision core values and customer service philosophy and adheres to the FH Compliance Program including following all regulatory requirements and the FH Standards of Behavior.

Supervises teaches and monitors adult and geriatric participants progress in all phases of the Preventive Cardiology & Rehabilitation programs.

Job Competencies and Essential Functions:

AED Accucheck POCT and other competencies as determined by need

  1. Clinical Duties:
    • Assesses program candidates for participation in the Preventive Cardiology & Rehab programs.
    • Conducts a clinical assessment on patients and develops an appropriate exercise prescription.
    • Monitors and supervises patients and evaluates their response to their exercise program.
  2. Patient Teaching:
    • Instructs patients on correct use of exercise equipment.
    • Develops patients educational program and/or needs through learning assessments and/or outcome measures; monitors effectiveness.
    • Conducts patient education lectures and/or one on one patient teaching as needed. Provides the necessary materials/handouts needed for effective communication and retention.
    • Leads Cardiac/Pulmonary group Orientations.
  3. Documentation:
    • Records all pertinent medical information per departmental and FHH policies; enters patient charges appropriately.
    • Communicates with the patients referring physician regarding patient progress and/or concerns within an appropriate time frame (this includes but is not limited to the patients initial treatment plan reassessment discharge summary and/or any changes/concerns in the patient while in the program).
  4. Quality Improvement/ Quality Assessment:
    • Establishes outcome measures; collects data; makes adjustments in the program and monitors these changes; reports outcomes to the department manager.
    • Participates in department annual goals and continuous quality improvement.
    • Participates in monthly staff meetings.
  5. Collaboration and Teamwork:
    • Works cooperatively as a team to service the patients. Shares information ideas and concerns constructively.
    • Fosters good inter-and intra-departmental relations.
    • Regularly on-time and follows the FHH Time and Attendance Policy.
  6. Quality:
    • Positive Customer Focus; attends to customers needs and develops positive rapport and communication with the customer.
    • Contributes in meeting the departmental patient satisfaction goals.
  7. Program Marketing:
    • As assigned helps to initiate and research avenues to increase participant volumes and promote the Preventive Cardiology and O/P Rehab programs via lectures health fairs physician/community outreach and etc.

Required Knowledge Skills and Abilities:

  1. Patient assessment skills effective communication and interpersonal skills patient teaching skills
  2. Knowledge of Cardiac/Pulmonary Anatomy and Physiology
  3. Telemetry monitoring and arrhythmia interpretation skills
  4. Working knowledge of exercise prescription preferred
  5. Knowledge of equipment i.e. exercise equipment emergency medical equipment audiovisual equipment routine office equipment

Minimum Education Training and Experience Required:

  1. Registered Nurse (RN) BSN preferred licensed in the state of Maryland
  2. One year experience in a critical care setting (Acute/sub-acute Cardiac/Telemetry Unit ER)
  3. Experience in a Cardiac/Pulmonary Rehabilitation setting preferred but not required
  4. CPR Certified
  5. ACLS Certified or obtained with 6 months of hire

Patient Contact

Must demonstrate and maintain current knowledge and skills in providing appropriate care/contact for patients in the following age groups:

  • Neonate (0 thru 30 days)
  • Infant (31 thru 12 months)
  • Child (13 months thru 12 years)
  • Adolescent (13 years thru 17 years)
  • Adult (18 years thru 65 years)
  • Geriatric (66 years)

Physical Demands:

Medium Work Lifting up to 50 pounds on an infrequent basis (less than one lift every three minutes) and/or carrying up to 25 pounds while walking. Pushing or pulling up to 75 pounds infrequently or up to 115 pounds occasionally (no more than once per hour).

Ergonomic Risk Factors:

Repetition: Repeating the same motion over and over again places stress on the muscles and tendons. The severity of risk depends on how often the action is repeated the speed of the movement the required force and muscles involved.

Forceful Exertion: The amount of physical effort required to perform a task (such as moving patients) or to maintain control of equipment or tools. The amount of force depends on the type of grip the weight of an object or person body posture the type of activity and the duration of the task.

Awkward Posture: Posture is the position your body is in and its effect on the muscle groups that are involved in the physical activity. Awkward postures include repeated or prolonged reaching twisting bending kneeling squatting working overhead with your hands or arms or holding fixed positions.

Contact Stress: Pressing the body hard on sharp edges can result in placing too much pressure on nerves tendons and blood vessels. For example using the palm of your hand to hit or stick objects can increase risk of musculoskeletal disorders.

Working Conditions:

Bloodborne Pathogens Exposure Risk: Category A WILL have exposure to blood or body fluids.

Reporting Relationship:

Reports to the Manager of Preventive Cardiology & Rehabilitation

Schedule:

Full time position 72 hrs per pay period
7p-7:30a with weekend and holiday rotation. 8hr on call per schedule

Pay is based on experience skills and education. If position is part-time salary will be pro-rated based on scheduled hours. The pay range may also vary within the stated range based on specialty if applicable. Non-Exempt positions may have shift differential and/or Overtime paid if applicable.

Pay range: $35.02/hr Min - $52.00/hr Max
