At the Gateway Medical Sobering Center the Nurse Practitioners (NPs) and/or Physician Assistants (PAs) play a vital role in managing risk around client biomedical conditions while exploring the appropriateness of Medication-Assisted Treatment (MAT) initiation. The NP/PA offers MAT as a clinical intervention to support individuals experiencing substance intoxication when elevated withdrawal symptoms and relapse-potential are present. Under the direction of the Medical Director the NP/PA assesses clients for MAT eligibility conducts health screenings and prescribes medications when clinically appropriate and within the appropriate scope of practice relative to the Medical Sobering programs design. When a client presents with medical conditions beyond the facilitys capacity the NP/PA initiates a timely referral to an appropriate healthcare provider or emergency services ensuring continuity of care and prioritizing client safety.

PRIMARY ACCOUNTABILITIES & RESPONSIBILITIES
- Support the peak utilization shift (typically Friday through Tuesday swing shift).
- Ensure swift on-site prescribing that doesnt depend on MD linkage after hours.
- Help deliver upon the projects withdrawal management capacity.
- Set clients up with medications needed to continue a withdrawal management episode post-discharge.
- Assess withdrawal severity using validated scales (CIWA COWS).
- Initiate prescribing of MATs and other medications.
- Delegate wound care functions to LVN or EMT on shift.
- Possess prescribing capacity for MATs and apply selected MAT based on patients level of functioning and drug use profile.
- Conduct assessments to determine appropriateness for Buprenorphine or other MAT remotely.
- Interprets vitals as collected by Staff and/or other healthcare assistant staff working within their scope of
Practice.
- Reviews health assessment information including communicable diseases to inform quality
clinical care during ones Sobering stabilization.
- Completes all documentation in an accurate and timely manner.
- Depending on patient acuity periodically observes residents for signs of mental or physical decompensation
that may require a higher level of care consulting with an MD as needed.
- Reviews withdrawal assessment scales (i.e. CIWA COWS etc.) leveraging results in order to safely stabilize
withdrawal symptoms through the prescribing of withdrawal management medications.
- Assists in collecting treatment appropriateness/fitness information for the Medical Director when needed
- Demonstrates an understanding of the pharmacological and physiological effects of alcohol and other
commonly abused substances
- Prescribes withdrawal management medications including medication-assisted treatment (MAT) under a
doctors supervision and records information about the current conditions of the patients
health.
- Reports medical incidents and unusual occurrences to the RN Program Manager Compliance Manager and
MD; supporting a continuous quality improvement process in order to mitigate risk in the future.
- Works closely with other HSI staff and referral resources to provide quality patient care communicating
and coordinating as a member of a multi-disciplinary treatment team which include EMTs LVNs RN Manager MD Clinical Supervisor Counselors & Peer Specialists.
- As an advanced-level healthcare practitioner serves as a coach and mentor to LVN EMT and other staff
- Records patient information the programs electronic health record and other record-keeping systems to case notes are complete and up to date according to agency policy
- Makes or receives calls related to client care as needed
- Provides support if a deviation from medication orders is requested
- Supports the program operation by offering guidance & mentorship to other staff
- Conducts and supports patient rounding targeting client linkages depending on acuity
- Develops and maintains positive working relationships with all staff members Agency and County
Contracts.
- Maintains attendance and punctuality to perform duties as scheduled.
- Attends all staff meetings and required agency meetings unless excused.
- Maintains a clean environment adhering to best practices
- Ensures to Review RN and LVN documentation as well as builds LVN and RN understanding of MAT
services.
- Other duties as requested by Medical Director and senior leadership team

What else is required
- New Mexico Nurse Practitioner (NP) or Physician Assistant (PA) License.
- 1-2 years of substance use disorder mental health or equivalent experience.
- CPR/First Aid/ AED current certification in New Mexico.
- Fluency in at least one of New Mexico threshold languages (Vietnamese Mandarin Cantonese Dari Arabic Swahili Kinyarwanda and Din (Navajo).
