Job Description

Job Title: Palliative Care Nurse

Work Location: Thunder Bay Ontario

Employment Type: Full-Time

SUMMARY

Reporting to the HCC Nurse Supervisor the Palliative Care Nurse collaboratively works with the Palliative Care Coordinator to assist in developing and implementing the palliative care program addressing the physical social emotional and spiritual care needs of clients and their caregivers. Palliative Care Nurse provides in-home nursing services works with a team of Personal Support Workers (PSWs) and other health care professionals. This individual will provide client centered nursing services to HCC clients in various KO communities. The palliative care nurse will fulfill responsibilities in accordance with the standards of practice and guidance of the College of Nurses of Ontario.

DUTIES AND RESPONSIBILITIES

1. Participate in the development and implementation of the palliative care program and standards of care with the HCC Palliative Care Coordinator and Nurse Supervisor. Coordinates and implements in-home nursing services under the direction of the HCC Nurse Supervisor and in conjunction with KO community Nurse in Charge and community Health Director.

2. Works with a team of Personal Support Workers (PSWs) unregulated healthcare providers and other healthcare professionals in the program.

3. Work collaboratively with the Home and Community Care team to proactively identify clients who are appropriate for palliative and end of life services and inform all referrals based on client/caregiver goals to determine the appropriate level and type of service.

4. Initiate arrange and facilitate case conferences with the interdisciplinary team as appropriate for management of client care issues such as determining families understanding of illness and discharge planning of palliative or end of life care needs.

5. Provide quality client focused care that enhances and promotes independence dignity comfort and wellness. Provide information and teaching of palliative and end of life care to clients and their caregivers. Perform personal support and personal care as required and as member of multi-disciplinary home care team. Perform wound care as per nursing assessment and demonstrated competency.

6. Maintain a relationship with the medical professionals in the community the Chief and Council and other stakeholders.

7. Maintain current skills through attendance at workshops in-service and staff meetings.

8. Assists the HCC Palliative Care Coordinator with preparing reports and annual year end reports for Indigenous Services Canada.

9. Performs other duties requested by HCC Nurse Supervisor KO Health Director or DON.

10. Charting using the Community Electronic Medical Record (cEMR) Telus.

KNOWLEDGE SKILLS AND ABILITIES

a. Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario.

b. Minimum 2 years of relevant experience in community home care or a related field.

c. Training/certification specific to palliative care (i.e. Fundamentals LEAP etc.).

d. Must have BLS or must be willing to obtain.

e. Knowledge on culture and tradition of remote Indigenous communities.

f. Excellent verbal and written communication skills. Ability to speak Ojibway Oji-Cree or Cree an asset.

g. Communicate effectively in both oral and written form. Be able to prepare and submit reports.

h. Commitment to ongoing learning and development of professionalism.

i. Basic computer skills and willing to learn a new cEMR system.

j. Awareness of working with unregulated Health Workers; Transfer/Delegation of Skills processes and requirements.

k. Ability to work independently or as a proactive team member.

l. Must have demonstrated ability to work with the Indigenous population as well as health and social service agencies and understanding cultural norms protocols and diversities.

m. Full time employment with the flexibility of weekends if required. Dependent on situation and any crisis situations. May be required to fly into the KO Communities.
