Internal Medicine/Geriatrics Permanent Role Location: Plano TX 75093
Shift: Full-time days 08:00 AM - 05:00 PM
Guaranteed Hours: 40 per week
Compensation: Based on experience
Key Responsibilities: - Deliver comprehensive geriatric assessments and primary care for high-risk adult and elderly patients
- Manage chronic conditions common in geriatric populations (e.g. dementia heart failure osteoporosis diabetes polypharmacy)
- Collaborate with a multidisciplinary team including advanced practice providers social workers pharmacists physical therapists and palliative/behavioral health specialists
- Build relationships with referring physicians and offer consultation regarding patient eligibility for geriatric care panels
- Promote functional independence and improved quality of life through individualized care planning
- Actively participate in quality improvement and utilization management initiatives specific to geriatric populations
- Maintain accurate and timely documentation in compliance with organizational standards
Qualifications: - Doctorate in Medicine (MD or DO)
- Licensed to practice in Texas
- Board-certified or board-eligible in Internal Medicine (ABMS or Bureau of Osteopathic Specialists)
- Geriatric fellowship training or significant experience in geriatric patient care
