Job Summary
UW Health in northern Illinois iis seeking a dedicated and compassionate Internal Medicine Advanced Practice Provider (ACNP AGANP or PA-C) to join our primary care team. As the preferred primary care provider in Northern Illinois UW Health offers a patient-centered approach to care focusing on comprehensive health and wellness for individuals across all stages of life. This position provides an exciting opportunity to work within a collaborative dynamic environment serving our diverse patient population.
Your clinical support includes the following:
- Providing primary care services for a wide range of adult patients with both acute and chronic conditions.
- Collaborating with a multidisciplinary team that includes physicians nurses care coordinators and specialists to provide holistic care.
- Managing patient care conducting physical exams performing diagnostic tests and interpreting results.
- Educating patients and families on managing chronic conditions and promoting preventive care.
- Participating in patient care planning and follow-up to ensure continuity of care.
Qualifications:
- Current and valid Illinois license as an Acute Care Nurse Practitioner (ACNP) Adult Gerontology Acute Care Nurse Practitioner (AGANP) or Physician Assistant (PA-C) or ability to apply for license prior to start
- Experience in Internal Medicine or a primary care setting preferred.
- Strong clinical decision-making diagnostic and communication skills.
