Job Description
Details for this part-time ACT Program position:
20 hours of patient care per week
Patient age is 26 years and older
Patients suffer from serious mental illness and have histories of hospitalization
Goal is to keep them out of the ED and prevent readmissions
Average of 60 patients within the program at any given time
Physicians go with therapists to patient homes within the community; Occassionally patients will come to the office location as well
Additional Psychiatric services offered by us but not within this providers job description include: An 18-bed medical complexity unit a 20-bed residential substance abuse detox and rehab 40-bed state-of-the-art inpatient psychiatric pavilion for adult patients a dedicated C/L service and extensive outpatient services throughout including partial hospitalization. Other services include Medication Assisted Treatment Behavioral Medicine Pain Management and a Senior Assessment Program.
