Hospice Case Manager (RN)
Hospice RN Needed in Monroeville PA

Salary and Benefits
- Salary: $70000 to $80000
- Health Insurance: Three Options to Choose From (Eligible 1st of the Month After 30 Days of Hire)
- Dental: Base Plan: $24.12 Per Month
- Vision: Top Tier Plan: $5.74 Per Month
- Accrued Paid Time Off: 4 Weeks After One Year
- 6 Paid Holidays
- Mileage Reimbursement - .58 Cents Per Mile

Responsibilities:
- Monday to Friday 8:30AM to 5:00PM
- Backup On-Call Only (Additional Compensation)
- Caseload: 15
- Assessments of Patients
- Pain Management
- Coordinate with Physician when needed.
- Anticipate changes and educate the patient family and caregivers on how to aid and manage symptoms safely.
- Completion of Annual Skill Checklist
Requirements:
- Licensed Registered Nurse in the State of Pennsylvania
- Coverage Area: Monroeville PA
- Preferred Experience: Hospice Home Health ER MedSurge Oncology
- Excellent Observation Verbal and Written Skills
- Reliable Transportation and Active Drivers License
