About the Role
To assist in the management coordination and development of a comprehensive county wide Team Support Service across Adults & Communities Care Pathway whilst maintaining a high-quality responsive customer service.
To work closely with designated teams in order to provide specialist support co-ordinating multi-agency support and assisting Care Pathway teams in the management of case work and the support of adult service users as appropriate.
To ensure that designated data sets are imported created and maintained in an accurate manner and produce high quality analytical dashboards in a range of formats including the TOM tracker to inform performance planning and reporting.
A DBS enhanced check for a regulated activity is required for this post.
This role is a Safety Critical Post and consequently subject to random drug and alcohol testing in accordance with our policies and procedures. If successful in being appointed to the role you will be required to agree to a planned test and achieve a negative result as part of any conditional offer of employment and/or during your probationary period. A copy of our Substance Misuse Policy and Procedure is available upon request.
