Job Description

TDC Case Manager
Position Status: Full-time
Classification: Exempt
Location: Tulsa Dream Center
Additional Information: This position is federally subsidized through a direct two-year grant established by the US Department of Housing and Urban Development (HUD)

SUMMARY
The YHDP (Youth Homelessness Demonstration Program) Caseworkers will serve to assess observe and monitor the welfare and housing related needs of underserved and identified at risk individuals through para-professional counseling and coordinating assistance with eligible needs as identified by HUD under the YHDP Program. This role will serve as the primary responsibility for all case management of the YHDP. Case workers will coordinate with each other Program Managers The Executive Director of TDC and other entities within the community providing various housing related services under the local Continuum of Care.
This position is grant funded and employment is contingent upon funding availability.

DUTIES AND RESPONSIBILITIES
  • Coordinate the process of intake and assessment of Youth and Young Adults (YYA) seeking client services.
  • Establish collaborative and supportive relationships with clients to assist with the assignment of appropriate services designed to prevent homelessness or urgent remediation to resolve related needs that may contribute to the permanent or temporary loss of housing.
  • Create populate and maintain confidential client digital records and files established to monitor and support client needs and services.
  • Provide consistent and caring support and care to all those seeking housing related assistance without bias to circumstances that may have contributed to interruption in stable housing.
  • Facilitate the screening process to ensure that proper identification is made for individuals who may be monitored under the DoC or other Federal Agencies for various reasons.
  • Consult with an interdisciplinary team to ensure appropriate referrals and diversion assistance has been provided within established policies and procedures as communicated by supervisors and Program leaders.
  • Observe all privacy and applicable HIPPA information that may disclosed during the assessment process to provide appropriate safety and security of protected personal information.
  • Schedule meetings and communications with other agencies and professionals as needs are identified.
  • Provide accurate and timely information to the Executive Director and the Grant Management team.
  • Facilitates the functional needs of the Youth Advisory Board (YAB).
  • Attend any mandatory meetings related to the YHDP Federal grant.
  • Participates in on-going in-service and professional development opportunities.
  • Follows instructions policies and procedures.
QUALIFICATIONS
  • Bachelors Degree in related field.
  • 1 year of relevant or related experience working within an agency or non-profit organization.
  • Familiarity and experience with Housing Related activities (or) Homelessness
  • Good communication problem solving and priority setting skills as well as maintaining an overall positive and professional attitude /disposition.
  • Ability to effectively plan organize and implement educational activities.
  • Ability to make decisions on behalf of children and protect their well-being.
  • Preferred experience with HUD endorsed Continuum of Care (CoC).

  • Sitting standing bending kneeling squatting crawling stooping walking and the ability to lift and carry up to 25 pounds and in case of emergency a 40-pound child;
  • Manual dexterity with the use of hands fingers to feel objects tools and controls;
  • Sighted with peripheral near and far vision;
  • Hearing and speaking;
  • Excellent communication skills.
