Nurse Practitioner (NP)

This community-centered clinic is redefining care with a holistic approach that combines medical excellence with lifestyle-focused services such as fitness programs art classes and wellness workshopsoffered free to patients.
Salary and Benefits
- Base Salary: $130000 with potential to earn over $143000 annually
- Performance Bonuses: Up to 10% of base salary based on satisfaction values HEDIS compliance and reduced hospitalizations

Responsibilities
- Provide outpatient primary care for seniors with multiple chronic conditions
- Operate in a Value-Based Care Model managing a panel of up to 650 patients
- Average 1012 patients per day
- Schedule: Monday to Friday 8 AM5 PM
- Participate in an on-call rotation (e.g. 1 in 32 weeks)
- Work with a dedicated support team: MA scheduler referral specialist and clinic leadership
Requirements
- Active and unrestricted Nurse Practitioner license
- Minimum of 2 years outpatient primary care experience
- Geriatrics experience is a plus
- DEA certification
- Competent in minor procedures: suturing injections skin biopsies and wound care
