Job Description
Burkwood Treatment Center is seeking a WI licensed Substance Abuse Counselor to provide treatment and support to assist patients in their recovery from addictions such as alcoholism and drug addiction.
**Group facilitation hours are 6:00 PM - 9:00PM 3 days per week**

ESSENTIAL JOB FUNCTIONS:
- Conduct individual and group counseling sessions in an residential treatment setting.
- Responsible for initial assessments as well as follow-up assessments for patients.
- Prepare individual treatment plan for each patient.
- Maintain patient evaluations notes and reports as required
- Assist with modifying problem behaviors and may also provide counseling services to families of patients.
- Provide crisis interventions as needed.
- Provide case management duties for patient ensuring individualized quality care.
- Evaluate the needs of the patient and determine if referrals to other programs or facilities is needed.
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
- High school diploma or equivalent required.
- Degree in social or health services field from an accredited college or university required.
- License in the State of Wisconsin as a SAC or CSAS required.
