Description

JOB SUMMARY: The Clinical Nurse Coordinator (CNC) is a registered nurse responsible for overseeing the coordination of care for participants enrolled in Redwood Coast PACE. The CNC ensures timely and appropriate clinical services are delivered across the care continuum including primary care specialty referrals care transitions and nursing interventions. This role works closely with interdisciplinary team (IDT) members to ensure that participant care plans are executed documented and aligned with regulatory requirements and the participants goals of care.

ESSENTIAL DUTIES AND RESONSIBILITIES:

  1. Serves as a liaison between nursing staff providers home care and other IDT members to ensure seamless clinical communication and coordination of nursing services.
  2. Monitor clinical workflows and nursing interventions to support timely follow-up on abnormal labs imaging and referrals and any other EMR in-basket items of their assigned reports.
  3. Coordinate with providers for care planning preventative care and urgent concerns addressing cultural and geographic differences in strategies and approaches.
  4. Participate in daily IDT meetings to provide clinical updates and recommendations based on participant needs and assessments.
  5. Oversee and ensure documentation compliance in the EMR in alignment with CMS DHCS and Redwood Coast PACE policies.
  6. Support and guide RNs and LVNs in clinical workflows and ensure adherence to nursing scope and standards.
  7. Coordinate on-call coverage among nursing staff.
  8. Monitors staff performance as it relates to competency of position/required training/compliance with Redwood Coast PACEs program and regulations.
  9. Facilitate participant transitions of care including post-hospital discharge ED visits and skills nursing stays ensuring follow-up care is timely and effective.
  10. Provide nursing assignments develop nursing care plans and delegate or coordinate interventions as needed.
  11. Support infection prevention efforts vaccine administration programs and chronic disease management initiatives.
  12. Monitor compliance with medication administration protocols and support pharmacy coordination when needed.
  13. Assist in auditing clinical documentation implementing quality improvement initiatives and training new clinical staff.
  14. Serve as a point of contact for urgent clinical issues when the provider is unavailable (Provider of the Day support as assigned).
  15. Travel to different site locations to check in with clinical staff and support clinic operations.
  16. Provide direct care to participants as backup when needed including home visits.

Qualifications

EDUCATION &/or EXPERIENCE:

  1. Minimum two (2) years clinical experience preferably in geriatric community health or primary care settings.
  2. Experience in case management care coordination or quality improvement preferred.
  3. Experience in and strong knowledge of nursing practice standards scope of practice and regulatory guidelines in a PACE environment.
  4. EMR experience preferred.

LICENSES OR CERTIFICATES:

  1. Must be a current RN licensed by the California Board of Registered Nursing in good standing.
  2. Current CPR and First Aid certification.
  3. Valid California Drivers License and personal auto liability insurance required. Must have a personal vehicle in good driving/operating condition and insured for State minimum liability requirements that can be used for the applicable job functions noted above.

PRE-SCREENING REQUIREMENTS:

  1. Requires clearance of a DOJ and FBI criminal history background check
  2. Pre-employment physical and PPD skin test.
  3. Requires Motor Vehicle Report verification to determine eligible driving record.
  4. COVID-19 and Influenza (flu) vaccines preferred.

SUPERVISORY REQUIREMENTS:

  1. This position supervises Registered Nurses (RN) and Licensed Vocational Nurses (LVN).
  2. In coordination with Human Resources and other supervisory staff oversees hiring orienting training evaluating disciplining and terminating of specified departmental staff.

PHYSICAL REQUIREMENTS:

The Clinical Nurse Coordinator position requires the ability to sit stand and walk for extended periods throughout the workday including movement between clinical areas and offices. Occasional travel to participant homes or off-site locations is required. The role involves frequent use of a computer and telephone as well as the ability to lift or move items up to 25 pounds. Must have the physical and sensory capacity to perform clinical assessments and respond promptly to urgent participant needs in a fast-paced environment.
