Position Description
PMC Mission and Values- Supervise and support registered nurses aides and other assigned home health staff.
- Manage daily scheduling to ensure consistent service availability.
- Conduct home assessments to evaluate client needs including medical functional psychological and environmental factors.
- Accept and manage referrals confirming eligibility and initiating reimbursement processes.
- Develop and oversee individualized plans of care in coordination with clients families and physicians.
- Perform skilled nursing tasks medication administration education and interdisciplinary referrals as needed.
- Maintain accurate and compliant documentation in the medical record.
- Oversee home health aide care planning and supervision.
- Participate in Quality Improvement and Professional Advisory Committees.
- Ensure policy and procedure manual is maintained and reviewed annually.
- Provide clinical guidance and oversight to RNs and CNAs.
- Collaborate with leadership to establish staffing patterns budget needs and service standards.
- Ensure physician involvement in plan-of-care development.
- Establish clear delegation of authority and communication within the program.
- Graduate of an accredited nursing program.
- Current Alaska RN licensure.
- 2-5 years of healthcare experience required; supervisory experience preferred.
- Strong communication leadership and clinical judgement skills.
- Knowledge of home health regulations and agency protocols.
