About the opportunity

We are seeking a compassionate and dedicated Mental Health Outreach Worker to support individuals with severe and enduring mental illness who face barriers accessing traditional services. This role delivers flexible person-centred and recovery-focused outreach in homes the community and other settings.

Key responsibilities include promoting wellbeing reducing isolation enhancing social inclusion and connecting participants with relevant services. Through assertive outreach and collaborative care planning you will help individuals maintain mental health stability and achieve personal goals.

This role supports the goals of the Engagement Hub day program by ensuring timely tailored support for those at risk of disengagement or facing complex challenges.
